About DigiSwasthya Foundation
DigiSwasthya Foundation is a healthcare-focused NGO working to make healthcare services affordable and accessible for rural communities across India through telemedicine and digital health solutions.
Position Summary:
The Community Health Mobilizer will be responsible for community engagement, awareness generation, and mobilizing rural populations to access healthcare services provided by DigiSwasthya Foundation. This position involves working closely with community members, coordinators, and healthcare providers to support the successful implementation of health-related projects and outreach activities.
Key Responsibilities:
    • Community Mobilization & Awareness:
    • Conduct awareness activities in villages and urban communities about telemedicine services offered by the NGO.
    • Educate community members on the benefits of telemedicine and preventive healthcare.
    • Inform community members about available health services, teleconsultations, and health camps.
    • Distribute information materials such as brochures, posters, and health education leaflets.
    • Encourage community participation in telemedicine consultations and health programs.
    • Patient Identification & Mobilization:
    • Identify individuals in the community who need medical consultation or health screening.
    • Encourage patients, especially vulnerable groups such as elderly, women, and children, to utilize telemedicine services.
    • Assist patients in reaching telemedicine centers or participating in health camps.
    • Coordinate with telemedicine center staff to ensure patients receive timely consultations.
    • Support follow-up visits for patients who require continued care.
    • Community Coordination & Stakeholder Engagement:
    • Coordinate with community leaders, local organizations, and health workers.
    • Work closely with government department and health workers for community outreach.
    • Facilitate communication between the DigiSwasthya, Hospital and community members.
    • Support organization of community meetings and health awareness sessions.
    • Strengthen community trust and participation in telemedicine programs.
    • Support for Telemedicine Operations:
    • Guide patients on the telemedicine consultation process.
    • Help patients with registration and appointment scheduling when required.
    • Assist telemedicine staff during health camps and outreach activities.
    • Provide basic assistance during teleconsultation sessions if required.
    • Ensure smooth coordination between patients and healthcare providers.
    • Reporting & Documentation:
    • Maintain records of community visits, awareness programs, and patient mobilization activities.
    • Submit regular reports on community outreach and patient referrals.
    • Track number of beneficiaries mobilized for teleconsultations and health camps.
    • Share feedback from community members to improve program services.
    • Support monitoring and evaluation of telemedicine program impact.

Qualification & Experience:

    • Graduate or degree in social work, community development, public health, or related field preferred.
    • At least 1–2 years of experience in community mobilization, preferably in a health or NGO setting.
    • Strong communication and interpersonal skills.
    • Fluency in local language(s) and familiarity with local customs and culture.
    • Passion for working with underserved or marginalized communities.
    • Ability to travel within project areas and work flexible hours, including weekends if needed.
