Job Overview

• Position Title: HCC Medical Coder (Hierarchical Condition Categories)
• Core Purpose: Ensure accurate coding of patient diagnoses and procedures to reflect health status, predict future healthcare costs, and support risk-adjusted payments.
• Work Environment: Hospitals, clinics, insurance companies, or coding service providers.

Key Responsibilities

• Medical Record Review
• Analyze physician notes, lab results, and patient charts.
• Assign ICD-10-CM and CPT codes aligned with HCC risk adjustment models.
• Risk Adjustment
• Capture chronic conditions and comorbidities that affect patient health and cost predictions.
• Optimize Risk Adjustment Factor (RAF) scores for Medicare Advantage plans.

Pay: ₹18,000.00 - ₹25,000.00 per month
