1. Area of expertise:

International Consultant - Primary care consultant, Doctor

2. Purpose of consultancy:

The World Health Organization (WHO) seeks to engage an international consultant with expertise in primary care and postgraduate family medicine/family practice training curricula development to support the Palestinian Ministry of Health (MoH) in the review and update of postgraduate training programmes in family medicine/family practice for doctors. This includes training curricula, accreditation pathways and recommendations for continuing professional development.

The consultancy aims to provide technical leadership to deliver the outputs.

3. Background:

The Ministry of Health (MoH) has requested the review and revision of national postgraduate training in primary care for doctors, to develop and promote the specialization in family medicine/family health as part of the multi-disciplinary primary care team. The revision aims to address gaps in primary care service delivery, workforce competencies, and responsiveness to emerging and growing health needs, including noncommunicable diseases (NCDs), mental health, and integrated, people-centred care. The content will enable primary care service delivery of the Essential Health Service Package.

The Palestinian health system in the West Bank operates within a complex and protracted emergency context characterized by political instability, movement restrictions, fiscal pressures, and service fragmentation. The shortage of specialized family practice doctors and nurses presents a major barrier to comprehensive, community-oriented care. The West Bank has a lower density of primary care providers relative to population needs, limiting access to preventive, diagnostic, and continuous care services. By investing in the training of primary care professionals, across disciplines, this project will increase the quality and comprehensiveness of primary care services, promoting a more resilient and equitable healthcare system that can respond effectively to community health needs. The consultancy will support MoH-led activities under WHO's technical assistance for primary care, improving the quality, accessibility and resilience of primary care in the West Bank.

Given the operating context in the West Bank, the project timelines may need to adapt with security and operational constraints.

4. Deliverables:

Output 1: Mapping and review of existing postgraduate training programmes in general practice, family medicine/family practice for doctors, in the West Bank

Deliverable 1.1: Inception Report

Inception report and desk review summary, including:

• A detailed mapping and analysis of postgraduate training landscape in primary care including programmes, training institutions, geographical distribution across north, middle and south regions of the West Bank, training content and institutional capacity

• A focused mapping of primary care and family medicine content in undergraduate curricula to identify the competencies doctors are expected to have before entering postgraduate training.

• Review of current curriculum content and modalities in comparison to international training models

• An overview of the current primary care doctor workforce

• Key informant interviews (MoH primary care focal points, training institution directors, primary care physicians).

• Proposed workplan and methodology for curriculum revision in partnership with selected institution, with timeline for development and validation.

• Recommendations for the national Primary Care Steering Committee

The inception report will inform the selection of the partner educational institution/s by WHO/MoH decision and propose a long-term institutionalization plan. The programme will be nationally stewarded by MoH, academically hosted and delivered by the selected educational institution, and accredited and recognized by the MoH and Palestinian Medical Council, in accordance with their respective mandates. Agreement on these arrangements will be prerequisite for curriculum development (Output 2)

Output 2: Development of a national competency based postgraduate specialty training programme and curriculum in family medicine/family practice

Deliverable 2.1: Postgraduate training curriculum

• 
  Participatory development approach working with partner educational institution

• Development of framework

• Formation of thematic groups for curriculum sections and provision of technical guidance

• Modular curriculum structure aligned with scope of practice and delivery of Essential Health Service package

• Submission of curriculum for necessary MoH and academic institution’s endorsement.

Deliverable 2.2: Infrastructure and equipment needs assessment for training delivery

• 
  Identify infrastructure limitations to deliver training, including equipment and technology support

Output 3: Development of a pathway for accreditation of family medicine/family practice in the occupied Palestinian territory

Deliverable 3.1: Consultation process (months 6-9)

• 
  Map and review existing mechanisms for accrediting educational institutions, postgraduate programmes and clinical training sites, and for certifying and licensing doctors and managing their CPD requirements.

• Design an accreditation pathway for the specialization, detailing the accreditation process, requirements and sequencing.

• Identify the steps required for operationalization within the health system, in collaboration with relevant stakeholders.

• Contribute to primary care workforce planning for family medicine

• Produce a policy brief to support adoption and an operational guide detailing the procedures, criteria, timelines, required documentation, review and inspection processes, renewal, quality assurance and appeals of the updated accreditation/licensing standards

Output 4: Produce recommendations for a continuous professional development (CPD) programme for family medicine/family practice physicians

Deliverable 4.1: Report on continuous professional development (CPD) models and recommendations

• 
  Describe the components of a continuous professional development system for practitioners in family medicine/family health

• Review and discuss models from comparable health systems

• Propose a national system for continuous professional development

• Identify challenges for implementation, and propose recommendations for piloting the CPD system in the oPt context

5. Qualifications, experience, skills and languages:

Educational Qualifications:

Essential:

• 
  First level University degree in Medicine.

• Advanced university degree in medicine, or Public Health.

• Postgraduate specialization in family medicine/family health/general practice.

Experience:

Essential:

• 
  Minimum of 10 years of clinical experience in family medicine/family practice/general practice.

• Documented experience in postgraduate training curricula development/ programme design for family medicine/ family health in one or more countries.

• Practical experience of delivering postgraduate training curricula.

Desirable:

• 
  Clinical experience in the Palestinian health system or similar context is an asse.

• Postgraduate training in medical education.

Technical skills and knowledge:

• Excellent knowledge of WHO PHC standards, UHC, EHSP, and family medicine/family health competencies.

• Strong expertise in postgraduate curriculum design, stakeholder facilitation, document formulation.

• 
  Excellent analytical, writing, communication, and organizational skills.

• Ability to work in sensitive contexts with cultural appropriateness.

Languages:

• 
  Proficiency in English

6. Location:

Remotely: planned trip to West Bank, Gaza Occupied Palestinian Territory for up to 4 weeks, subject to security conditions.

7. Remuneration and budget (Travel cost not included):
