Job Responsibilities

• Perform medical transcription and documentation.
• Enter and update patient information in EMR/EHR systems.
• Maintain patient medical histories, diagnoses, medications, and allergies.
• Prepare physician notes, progress notes, and clinical documentation.
• Manage patient demographics and insurance details.
• Understand medical terminology and clinical abbreviations.
• Ensure accuracy and confidentiality of patient records.

Required Skills

• Minimum 2 years of experience in Medical Transcription or EMR/EHR Data Entry.
• Knowledge of medical terminology.
• Experience in medical chart documentation.
• Good English communication and typing skills.
• Knowledge of US healthcare documentation and HIPAA guidelines.
• Strong attention to detail and accuracy.
