Job description

Role & responsibilities

• Transcribe medical reports, consultation notes, discharge summaries, operative notes, and other clinical documents accurately and within defined timelines.
• Listen to physician/clinician dictations and convert them into clear, grammatically correct written documents.
• Ensure correct spelling and usage of medical terminology, drug names, procedures, diagnoses, and anatomical terms.
• Review and proofread transcribed documents for accuracy, completeness, formatting, and consistency.
• Maintain appropriate medical terminology, abbreviations, and hospital documentation formats.
• Identify unclear or incomplete dictations and seek clarification from the concerned clinician when required.
• Maintain strict confidentiality of patient information and comply with hospital privacy and data-security policies.
• Ensure timely completion and submission of assigned transcription work.
• Maintain proper records of completed reports and follow departmental documentation procedures.
• Coordinate with Medical Records, clinicians, and other relevant departments as required.
• Support internal audits and quality initiatives related to medical documentation.
