Case Manager responsibilities and duties

- Communicate regularly with residents and their families about Plans of Care, Physio Therapy (PT), Occupational Therapy (OT) and other treatment protocols

- Devise procedures and protocols for patient admission and filtration so that each resident receives consistent, attentive care through the duration of their stay with us

Case Manager qualifications and skills
